    Almost all counselors at Breakthrough are in-network with Cigna. We also accept Cigna EAP.

    All counselors at Breakthrough are in-network with commercial Blue Cross Blue/Shield plans, including Anthem. We are NOT in network with BCBS EAP plans.

    Roger and Rochelle are in-network with United Healthcare, United Behavioral Health, UMR, Optum and Oscar.

INSURANCE INFORMATION

Your medical insurance plan is a form of compensation based on a contract between you, your employer and the insurance company. You can expect the carrier (insurance company) to pay for a portion of our fee. That portion is determined by the contract between your employer and the insurance company. The higher the premium paid by your company, the more generous the benefits. We will do our best to familiarize ourselves with the details of your coverage, but as each individual plan is unique, it is ultimately your responsibility to understand your specific benefits.

Although we are not a party to the contractual arrangement with your insurance company, we do want to help you receive the maximum discount or reimbursement to which you are entitled. As a convenience to you we will help you submit your insurance claims for you to receive this maximum benefit.

At all times, you can be confident that we will provide you with our best services without regard to the limitations imposed by your insurance coverage.

Not all services are covered benefits with the insurance. Some insurance companies select certain services they will NOT cover, such as couples or family counseling. It is ultimately the patient’s obligation to know and understand his/her insurance policy.

We must emphasize that, as mental healthcare providers, our relationship is with you, the responsible party, not your insurance company. While the filing of insurance claims is a courtesy that we extend to our patients, all charges are your responsibility from the date the services are rendered. We realize that temporary financial problems may affect timely payment of your account. If such problems do arise, we encourage you to contact us promptly for assistance in the management of your account.

PLEASE NOTE: PAYMENT IS DUE AT THE TIME OF SERVICE.

As a courtesy to you, we will bill your insurance company and track claims. Please keep us informed of any changes to your insurance plan. You are responsible for the fees charged by our office, no matter what your insurance coverage may be. Most insurance companies should respond to the claim within four to six weeks. Any remaining cost is your responsibility.

 

SELF-PAY RATES FOR UNINSURED CLIENTS:

Group Therapy Sessions are 60-90 minutes and are $30/session

 

Individual/Couples Sessions are 45-60 minutes

Master’s Degree Intern: $75/session

Licensed Associate Counselor: $95/session

Licensed Professional Counselor: $150/session

Breakthrough is not in network with AHCCCS, Medicare, or Tricare
